We work with you to agree practical support that can build confidence, help overcome barriers and make progress towards what matters to you.
We help you develop skills and confidence to manage everyday life, building on what you can already do.
We provide practical community support for adults experiencing isolation, reduced confidence or difficulties participating in everyday life.
We work alongside adults whose disability or long-term condition affects everyday activities or participation in community life.
Supporting someone else can affect your own wellbeing, relationships and opportunities. We help unpaid carers identify their priorities and explore support for themselves.
We work with commissioners and community partners to develop and deliver prevention and wellbeing programmes that respond to identified local needs.

How we support you
Our support is practical, personalised and reviewed with you over time.
We help engage adults who face barriers to accessing support and take agreed first steps towards relevant services.
We consider strengths, choices, communication preferences, access needs and relevant risks before agreeing activities and responsibilities.
Where appropriate, we work with families, representatives, professionals and community organisations while keeping your wishes and privacy central.
We review what is working and what needs to change, focusing on confidence, connection, access and independence.
How support begins
Tell us what you are looking for, where support is needed and who you are enquiring for.
We consider service scope, availability, immediate risks and the information needed to understand the next step.
Where suitable, we discuss goals, proposed activities, responsibilities, costs or funding and review arrangements.
Support begins once arrangements are agreed. We review progress with you as your needs and priorities develop.

For professionals and commissioners
We welcome professional referrals and conversations with commissioners, health and social care organisations, and community partners.
Before delivery begins, we work with referring or commissioning organisations to clarify eligibility, service suitability, outcomes, responsibilities, funding, reporting and review arrangements.
Costs and funding
The cost of support depends on the service, level and frequency of support, location and agreed delivery arrangements.
Following an initial discussion, we will explain any applicable charges and provide a quotation for suitable support. Before you commit, we will clarify what is included, any additional costs, payment terms and cancellation arrangements.
Support may be privately funded or delivered through an agreed commissioning arrangement. Where another organisation is funding your support, we will clarify the arrangement and any contribution you may be required to make.
